Swollen Ankles Have Several Causes, Not All Cardiac
Ankle swelling is produced by pressure, protein levels, vein and lymph drainage or medication, and distinguishing between them depends on pattern, symmetry and what else is happening.

Swollen ankles are commonly assumed to indicate heart trouble. Several distinct mechanisms produce the same appearance, and telling them apart depends on pattern rather than on the swelling itself.
The underlying physics is the same in every case
Fluid moves out of capillaries into tissue driven by pressure inside the vessel, and is drawn back by proteins in the blood that exert an opposing osmotic pull.
Whatever remains in the tissue is returned by the lymphatic system. Swelling appears when outflow exceeds return, whichever part of that balance has changed.
Gravity concentrates the effect at the lowest point, which is why ankles swell in people who are upright and why the sacrum swells in people who are not.
Raised venous pressure
When the right side of the heart cannot move blood forward efficiently, pressure backs up through the veins and capillary pressure rises throughout the lower body.
Chronic vein disease produces the same local effect without any heart problem, because damaged valves allow blood to pool in the leg veins on standing.
Simple immobility contributes as well, since the calf muscle pump normally drives venous return and stops working when the legs are still for long periods.
Low blood protein and drug effects
Reduced albumin, whether from liver disease, kidney protein loss or prolonged undernutrition, lowers the osmotic pull holding fluid inside vessels.
Several drug classes cause swelling directly, including some blood pressure medicines that dilate small arteries and raise capillary pressure downstream.
Drug-related swelling typically begins within weeks of starting or increasing a medicine, which is the detail that identifies it, and it does not respond well to diuretics.
Lymphatic drainage and skin changes
When lymphatic drainage is impaired, the swelling is characteristically firmer, often involves the toes, and pits less readily than fluid from raised pressure.
Long-standing swelling of any cause damages the skin, producing thickening, discolouration and vulnerability to breakdown and infection.
Cellulitis presents as swelling that is one-sided, red, hot and painful, which is a different situation requiring prompt medical attention.
What distinguishes them in practice
Symmetry is the first question. Swelling on one side suggests a local cause such as clot, infection or vein obstruction; both sides suggests a systemic one.
Accompanying breathlessness, particularly when lying flat or waking at night, points toward a cardiac cause and changes the urgency considerably.
New swelling, one-sided swelling, or swelling with breathlessness or chest pain needs assessment rather than elevation and compression alone.
