The Hepatitis C Screening Most Adults Never Expected
Hepatitis C screening was widened from risk-based testing to a one-time test for adults because most infections are silent for decades and cured once identified.

Hepatitis C screening moved from a risk-based approach to a recommendation that adults be tested once. The change followed from two facts about the infection that make it unusual.
The infection is silent for a very long time
Most people who acquire hepatitis C have no symptoms at the time and none for years afterward. The virus establishes chronic infection in a majority of cases without announcing itself.
Liver damage accumulates slowly and quietly. Fibrosis progresses over decades before it produces fatigue, swelling or the laboratory changes that would prompt investigation.
By the time symptoms appear, the damage may be advanced. That long silent interval is exactly the window in which testing has value.
Risk-based testing missed too many people
Earlier guidance asked clinicians to test people with identified risk factors, including past injection drug use and transfusions received before blood supply screening existed.
That approach depended on the risk being disclosed and remembered. Exposures decades in the past are easy to forget, and some carry enough stigma that people decline to mention them.
Clinicians also had to raise the topic, which happened inconsistently. A universal one-time test removes both the disclosure step and the judgment about who to ask.
Why treatment changed the calculation
Older treatment was long, poorly tolerated and often unsuccessful. Screening for a condition that could not be reliably treated offered limited value against considerable distress.
Direct-acting antiviral therapy changed that. Treatment courses became short oral regimens with high cure rates, which converted a diagnosis into something actionable.
Screening recommendations follow treatment availability more often than people assume. Finding a disease is only worthwhile when finding it changes what happens next.
How the testing sequence works
The first test looks for antibodies, which indicate exposure at some point. A positive antibody result does not distinguish current infection from an infection that resolved.
A second test looks for viral genetic material and answers whether the virus is still present. Only that result establishes active infection needing treatment.
People sometimes receive the antibody result alone and assume the worst. The two-step structure is standard, and the second test is what carries the meaning.
What a diagnosis leads to
Assessment of liver scarring follows, using blood-based indices or elastography rather than biopsy in most cases. That staging determines whether monitoring continues after treatment.
Cure eliminates the virus but does not reverse established cirrhosis, which is why the timing of detection matters so much for outcomes.
Anyone with a positive result should be referred for evaluation rather than reassured by feeling well. Absence of symptoms is the characteristic feature of the infection, not evidence against it.
