What A Coronary Calcium Score Actually Measures
A calcium score counts calcified plaque in the heart's arteries, which reflects accumulated disease rather than current narrowing, and its usefulness falls as almost everyone scores above zero.

A coronary calcium score is a single number from a brief scan. Understanding what it counts, and what it cannot see, determines how much weight it deserves.
What the scan detects
A low-dose computed tomography scan of the chest identifies calcium deposits in the walls of the coronary arteries, without any injected contrast.
Software quantifies the area and density of each deposit and combines them into a total score, which is then compared with reference values for age and sex.
The scan takes minutes and involves a modest radiation dose, which is one reason it became widely available for risk assessment.
Calcium marks healed plaque, not active narrowing
Atherosclerotic plaque calcifies as part of a healing response, so calcium indicates that plaque has been present long enough to stabilise.
The score therefore reflects the accumulated burden of disease over time rather than how narrowed any artery currently is.
A high score does not indicate an obstruction, and a person with substantial calcium may have entirely adequate blood flow through those vessels.
Soft plaque is invisible to it
Early, lipid-rich plaque without calcium does not appear on the scan, and this is the type most associated with sudden rupture.
A score of zero therefore indicates low risk over the following years but is not a guarantee, particularly in younger people or heavy smokers.
This limitation is the main argument for not treating the number as a definitive statement about the state of the arteries.
Why it becomes less useful with age
Calcium accumulates over decades, so the proportion of people with a score above zero rises steadily and approaches most of the population in later life.
A test that almost everyone fails discriminates poorly, which is why the score adds less to risk assessment in older adults than in middle-aged ones.
Its clearest use is in intermediate-risk people where the decision about preventive treatment is genuinely uncertain and an additional input would change it.
What follows from a result
A score does not by itself indicate a procedure. Investigating symptoms and assessing blood flow are separate questions requiring different tests.
What it can change is how intensively conventional risk factors are managed, since a high score reclassifies someone into a higher risk category.
Whether the scan is worth having, and what should follow, depends on the individual risk profile and belongs in a discussion with a clinician rather than being ordered speculatively.
