What Routine Blood Tests Do And Do Not Screen For
A standard panel measures a narrow set of markers chosen for cost and convenience, and a normal result excludes far less than most people assume it does.

An annual blood test is often treated as a general check for hidden illness. A standard panel measures a specific and narrow set of things, and a normal result excludes less than assumed.
What a standard panel contains
A full blood count measures red cells, white cells and platelets, detecting anaemia, some infections and some blood disorders.
A metabolic or biochemistry panel covers kidney markers, electrolytes, liver enzymes, glucose and often calcium, giving a broad view of organ function.
Lipids and thyroid function are frequently added, and each of these is a specific measurement chosen because it is cheap, stable and interpretable.
What it does not look for
Most cancers produce no abnormality on a routine panel until late, and the markers that do exist are generally used for monitoring known disease rather than for screening.
Vitamin and mineral status is largely absent unless specifically requested, and several nutrients have no reliable blood measure of tissue stores.
Structural problems are invisible entirely. Narrowed arteries, valve disease, joint damage and early cognitive change do not appear in blood chemistry.
Reference ranges are not the same as health
A reference range is typically the central span of results from a reference population, which means a proportion of healthy people fall outside it by definition.
Ranges are often derived from younger adults, and several markers shift predictably with age without indicating disease.
A result just outside a range therefore requires interpretation in context, and a result well inside one can still represent a change from that person's own baseline.
Trends carry more information than single values
A kidney marker stable at the upper limit for years means something different from the same value reached after a steady rise.
This is why clinicians look at previous results, and why testing at a familiar laboratory helps, since assay methods differ between providers.
It is also why repeating an isolated mildly abnormal result is often the appropriate next step rather than immediate investigation.
The harms of testing without a question
Any panel of measurements produces occasional out-of-range results by chance, and each one can trigger further testing with its own risks and costs.
Incidental findings generate anxiety and surveillance that may never have been warranted, which is why testing without a clinical question is discouraged.
The productive approach is asking what a test is intended to answer and what would change depending on the result, which is a conversation with the requesting clinician.
