Why Aspirin Guidance For Older Adults Was Revised
Daily aspirin for prevention was once near-universal advice for older adults, and the reasoning changed because bleeding risk rises with age faster than the benefit does.

Daily low-dose aspirin was, for a long stretch, standard advice for older adults who had never had a heart attack. That recommendation narrowed substantially, and the reasoning is worth understanding.
Two different uses share one name
Secondary prevention means taking aspirin after a heart attack, stroke or stent, where the aim is to prevent a second event. Primary prevention means taking it before anything has happened.
The evidence for those two situations is not the same, and it never was. Advice that changed applies to the second case, which is where most healthy older adults sit.
Confusing the two is common and consequential. Anyone with existing cardiovascular disease should not change what they take on the basis of general reading, and that decision belongs with their clinician.
How aspirin works and why that cuts both ways
Aspirin permanently disables an enzyme in platelets, reducing their ability to clump. Since platelets cannot repair the damage, the effect lasts for the life of the cell.
Clumping is what turns a ruptured arterial plaque into a blocked artery. Suppressing it is protective in exactly the circumstances where a clot would otherwise form.
The same suppression makes any bleeding harder to stop. There is no mechanism that separates useful clotting from harmful clotting, so the benefit and the hazard travel together.
Bleeding risk climbs with age
Older stomach lining is more vulnerable, more people take other drugs that irritate it, and falls become more frequent. Each of those raises the chance that suppressed clotting causes harm.
Bleeding inside the skull is the outcome that weighs heaviest, because it is uncommon but severe. Its likelihood rises with age in a way that gradual clotting benefit does not offset.
The result is a curve that crosses over. In a healthy person with low cardiovascular risk, the expected harm eventually exceeds the expected protection.
Why the advice felt like a reversal
Earlier guidance was built on trials conducted before widespread use of blood pressure treatment and cholesterol-lowering therapy. Background risk was higher then, so the same drug bought more.
As other prevention improved, the incremental value of aspirin shrank while its bleeding risk stayed constant. Nothing about the drug changed; the population around it did.
That pattern recurs across preventive medicine. A recommendation is not wrong when it changes, but it is tied to conditions that can move underneath it.
What this means in practice
Starting aspirin for prevention is now treated as an individual decision rather than a default, weighing personal cardiovascular risk against bleeding history and other medications.
Stopping abruptly is its own question, particularly for anyone taking it after a cardiac event. That is a conversation for the prescriber, not a change to make quietly.
The broader lesson is that a cheap, familiar, over-the-counter drug can still carry real trade-offs, and familiarity is not evidence of safety.
