Alcohol after 60: the same drink does more

Body composition, liver metabolism and medication all change how alcohol behaves. The result is that a lifelong habit can become a problem without the amount changing at all.

People rarely increase their drinking in later life. What changes is what the same amount does to them, and the change is larger than most realise.

Why the same glass hits harder

Body water falls. Alcohol distributes into total body water, which declines with age as lean mass is replaced by fat. The same quantity of alcohol in a smaller volume of water produces a higher blood alcohol concentration — for an identical drink, identical body weight and identical circumstances. The difference between a seventy-five-year-old and their thirty-five-year-old self can be substantial.

Hepatic metabolism slows. Liver blood flow and enzyme activity both decline, so alcohol is cleared more slowly and the exposure lasts longer.

The brain is more sensitive. Older adults show greater impairment of balance, reaction time and cognition at a given blood alcohol level than younger adults.

Put together, a person who has drunk two glasses of wine every evening since their forties is, at seventy-five, experiencing something meaningfully stronger than they were — without having changed anything.

The interactions, which are the bigger risk

This is where most of the actual harm lives, and it is the part least often discussed.

  • Benzodiazepines and z-drugs — additive sedation, markedly increased falls and confusion risk.
  • Opioids — additive respiratory depression, which can be fatal.
  • Antihistamines and other sedating drugs.
  • Paracetamol — regular heavy drinking increases hepatotoxicity risk at therapeutic doses.
  • NSAIDs — combined gastrointestinal bleeding risk.
  • Warfarin — alcohol affects its metabolism, and binge drinking in particular destabilises INR.
  • Metformin — increased lactic acidosis risk with heavy intake.
  • Antihypertensives — additive postural hypotension.

Given that a large fraction of adults over seventy take five or more medicines, the probability of at least one meaningful interaction is high, and it is rarely raised at a prescribing appointment.

What the guidance says now

Most countries have converged on a limit of around 14 units per week (roughly six pints of average-strength beer or six medium glasses of wine), spread over three or more days, with several alcohol-free days. Some bodies advise lower thresholds for adults over 65 — the US National Institute on Aging suggests no more than one standard drink a day. There is no consumption level established as risk-free.

The cardiovascular claim, revisited

For decades, moderate drinking was reported to be cardioprotective, on the basis of a J-shaped curve in observational data — light drinkers appearing to fare better than abstainers.

That finding has weakened considerably under scrutiny. The main problem is the sick quitter effect: abstainer groups in these studies included people who stopped drinking because of illness, making abstainers look unhealthy for reasons unrelated to alcohol. Analyses that separate lifelong abstainers from former drinkers attenuate or abolish the benefit.

Mendelian randomisation studies, which use genetic variants affecting alcohol metabolism as a natural experiment and are less vulnerable to this confounding, generally find a linear relationship between alcohol and cardiovascular harm rather than a J-shape.

The honest position is that any cardiovascular benefit of moderate drinking is smaller than was believed and may not exist, while the harms — cancer in particular — are linear with dose from low levels.

Cancer, which is under-communicated

Alcohol is classified as a Group 1 carcinogen by the International Agency for Research on Cancer. It is causally associated with cancers of the mouth, pharynx, larynx, oesophagus, liver, colon, rectum and breast.

The breast cancer association is dose-dependent from low intakes, with risk rising measurably even within recommended limits — a fact that public awareness surveys consistently show almost nobody knows.

The specific risks in later life

Falls. The most immediate. Alcohol impairs balance and reaction time in a population already at risk, and often in the evening, in dim light, on the way to the bathroom.

Cognition. Heavy drinking is an established risk factor for dementia, and the Lancet Commission includes excessive alcohol among modifiable risk factors. Alcohol-related brain damage, including Wernicke–Korsakoff syndrome from thiamine deficiency, is under-diagnosed and sometimes partially reversible if caught.

Sleep. Alcohol shortens sleep latency and then fragments the second half of the night, suppressing REM. Using it as a sleep aid produces worse sleep, and the pattern is common.

Depression and isolation. Drinking often increases after bereavement or retirement, and late-onset problem drinking — beginning after sixty in someone with no prior history — is a recognised pattern that is easily missed because it does not fit the stereotype.

Cutting down

Two practical points. First, if someone has been drinking heavily for a long time, stopping abruptly can be dangerous — alcohol withdrawal causes tremor, agitation, seizures and delirium tremens, and in older adults it is more severe. Reduction should be medically supervised where intake has been substantial.

Second, the useful interventions are ordinary: measure what you actually pour, because home measures are typically far larger than standard units; establish drink-free days as a rule rather than an intention; do not keep spirits in the house if spirits are the problem; and address what the drinking is doing — for a great many people in this age group, it is managing loneliness, boredom or insomnia, and none of those is solved by removing the alcohol alone.

Raise it at your next medication review. A GP asked directly about alcohol and drug interactions will usually have something useful to say, and it is a conversation that is rarely initiated from the other side of the desk.
