Caffeine is cleared more slowly with age and interacts with several common medicines, which is why the same afternoon coffee starts affecting sleep and blood pressure differently.

Many people find that coffee affects them differently in later life without having changed how much they drink. The reasons involve clearance rate, medication interactions and sleep structure.

How caffeine is cleared

Caffeine is broken down almost entirely in the liver by a specific enzyme, and the rate at which it works varies substantially between individuals for genetic reasons.

Liver blood flow and enzyme activity both decline modestly with age, so the time taken to clear a given amount lengthens.

A longer clearance time means a mid-afternoon drink can still have meaningful concentrations present at bedtime, which was not previously the case.

Why sleep is affected more than before

Caffeine blocks receptors for adenosine, the molecule that accumulates during waking and generates sleep pressure. It masks tiredness rather than supplying energy.

Deep slow-wave sleep already diminishes with age, and caffeine reduces it further, so the same dose removes a larger proportion of what remains.

The resulting tiredness is then commonly met with more caffeine, which is a self-sustaining pattern rather than a solution to the underlying problem.

Interactions with medication

The enzyme that clears caffeine also handles several drugs, so some medicines slow caffeine clearance and extend its effects considerably.

Caffeine also interferes with the absorption of certain drugs when taken at the same time, which is why some medicines specify water rather than any hot drink.

Because these interactions are specific rather than general, they are a question for a pharmacist reviewing the actual medication list.

Cardiovascular effects are modest but real

Caffeine produces a short-lived rise in blood pressure, more pronounced in people who consume it infrequently than in habitual drinkers.

It can also increase awareness of palpitations, though moderate habitual intake has not been shown to cause serious rhythm disturbance in most people.

Where blood pressure is being monitored, drinking coffee shortly before a reading distorts it, which is why measurement guidance usually specifies avoiding it beforehand.

The other effects worth knowing

Caffeine has a mild diuretic effect, which matters more when thirst signalling is already less reliable and intake is marginal.

Tannins in tea and coffee reduce absorption of iron from plant sources when consumed with a meal, which is relevant to people with low iron stores.

Abrupt cessation produces headache and fatigue for several days, so anyone reducing intake for a medical reason generally does it gradually and on advice.
