Getting Out Of A Car Is A Complicated Movement
Rising from a car seat combines a low seat height, a rotating trunk and a doorframe obstruction, which is why it fails earlier than standing up from a chair.

People who stand from a kitchen chair without difficulty can find a car seat defeats them. The car adds constraints that a chair does not, and each one raises the demand.
Seat height is the first constraint
Rising force required grows sharply as the starting hip position drops. A low car seat can put the hips well below knee height, which is where the sit-to-stand becomes hardest.
Sedans place the hips low relative to the feet, while taller vehicles put them closer to standing height. The difference in required strength between the two is considerable.
This is why people trading down to a smaller car sometimes discover that a vehicle they can afford is one they cannot easily get out of.
The movement requires rotation under load
Exiting a car means swinging the legs out of the footwell while the trunk turns, then standing. Rotation and rising are combined rather than sequential.
Trunk rotation under load is limited by hip mobility and spinal flexibility, both of which narrow with age. Restricted rotation forces the whole body to shuffle around.
Doing it in two clear stages, turning fully to face outward with both feet planted before rising, removes the combination and usually resolves the difficulty.
The doorframe removes the natural strategy
Standing from a chair normally involves leaning the trunk forward to bring the body's weight over the feet. That forward lean is what makes the movement possible.
A car doorframe sits directly in the path of that lean, so people rise more upright than they should. The result is a movement that demands far more from the legs.
Sliding forward on the seat before standing achieves some of the same effect, because it moves the hips closer to the feet without requiring the lean.
Where the hands go matters
Pulling on the door is the common instinct and the least stable option, because the door moves. It also puts a shoulder under load in an awkward position.
The doorframe above, the seat itself, or a handle mounted to the door latch provides a fixed point. Pushing down from the seat is generally more effective than pulling up.
Small grab handles designed for this exist and are inexpensive. They restore a fixed anchor without modifying the vehicle.
Why the difficulty is worth reporting
Trouble exiting a car often precedes trouble with other transfers, because it is the most demanding one most people perform daily.
It also determines whether someone continues to accept rides, attend appointments and see people. The consequence is social before it is physical.
Mentioning it to a clinician or physical therapist is worthwhile, since the underlying capacity is usually trainable and the alternative is gradually staying home.
