What Water Exercise Changes And What It Hides
Buoyancy removes joint load and makes movement possible for painful joints, but it also removes the bone and balance stimulus that land-based work provides.

Pool exercise is recommended constantly for older adults with painful joints, and for good reason. What it offers and what it cannot offer both follow from the same physics.
Buoyancy removes weight from joints
Immersion offsets a large part of body weight, and the proportion rises with depth. Standing chest-deep reduces the load through the knees and hips substantially.
That unloading is what allows someone with severe knee arthritis to walk comfortably in a pool while struggling on land. The joint is doing far less work.
Range of motion often improves in water for the same reason, because movement is limited by pain rather than by the joint's actual capacity.
Water provides resistance instead
Moving a limb through water meets drag that rises steeply with speed. Pushing faster produces more resistance without adding any equipment.
The resistance is self-limiting, which makes it safe. A movement cannot be accelerated into a position that overwhelms the joint the way a dropped weight can.
It also acts in every direction rather than only against gravity, so muscles on both sides of a joint work during the same movement.
What the water takes away
Bone responds to impact and to load, and buoyancy removes both. Pool exercise does not provide the mechanical stimulus that maintains bone density.
Balance training is similarly diminished. Water supports the body and slows any loss of balance, so the rapid correction that land-based balance work trains never has to occur.
Someone whose entire program is aquatic can therefore improve fitness and comfort while their fall risk and bone health remain unaddressed.
The heart works differently underwater
Immersion pushes blood from the limbs toward the chest, increasing the volume the heart handles with each beat. Heart rate at a given effort is typically lower than on land.
That makes heart rate a misleading gauge of intensity in the pool, and perceived exertion a better one. People frequently assume they worked less than they did.
The central blood shift also increases urine production, which is why a session often produces a strong urge to use the bathroom afterward.
Where it fits in a program
Water work suits painful joints, periods of recovery, and anyone whose land-based activity has stopped because it hurts. It restores movement when movement has become impossible.
Pairing it with even modest weight-bearing activity covers the gap it leaves. Walking and standing balance work supply what buoyancy removes.
A physical therapist can advise on that balance for a specific joint problem, particularly after surgery, where timing for pool entry depends on wound healing.
